Item 709 (GP) Item 711 (PN) The check must be delivered in conjunction with the 4 year old immunisation.
The Get Set 4 Life Booklet can be ordered from DoHa at resourcekits@health.gov.au The Healthy Kids Item number CAN NOT be claimed if the 4 year old immunisation has not been given, or with another GP attendance on the same day. 45 Year Health Check

The Australian Better Health Initiative aims to enhance the capacity of the health system to promote good health and reduce the burden of chronic disease.It will support GPs to provide a health check to patients who are between 45 and 49 who are at risk of developing a chronic disease.

Asthma 3+ Visit Plan has been renamed the Asthma Cycle of Care

The Asthma initiative is for patients with moderate to severe asthma who in the opinion of the doctor could benefit from review e.g. management could be improved.
